Upon entering our office, our staff will welcome you as a member of our family. Click the button below to complete our simple and easy to fill, new patient forms. A PIN will be required to fill out the forms.

Next, you will have a consultation with Dr. Allen to discuss your health-related problems, concerns, and potential treatment options. This initial visit is designed for Dr. Allen to learn more about you, your condition, and expectations to determine how chiropractic care can meet your goals.
After your consultation, Dr. Allen will perform a complete chiropractic examination analyzing your location of pain, including tests to determine motion and function. Other standard neurological, orthopedic, postural, and physical tests may be performed as well. However, nothing will be done in our office without your prior consent.
Depending on your specific condition, Dr. Allen may indicate that X-rays are needed. X-rays help us develop the most effective treatment plan for you, and alert us of any serious spinal conditions. Most people are amazed once they see their X-rays up close. In some cases, people can easily spot the areas causing their discomfort.
At the conclusion of your initial appointment, we offer a relief treatment to help you on your way to feeling your best. This may include spinal adjustments, stretching, muscle release therapy, and/or soft tissue massage. Treatment is interactive, so you can express concerns about the different styles of treatment anytime.
Most insurance plans are accepted at our office. We accept auto accident, workers compensation, personal injury and most health insurance plans, including Medicare and Medicaid. Since there are so many plans and coverage varies widely, please contact us to find out if your plan covers care in our office. Our staff will be happy to verify and explain your benefits before treatment.
Pure Wellness Chiropractic + Acupuncture believes in the value of wellness care for you and your family and have maintained a pricing structure that allows care for all budgets. For patients who have little or no chiropractic insurance coverage, flexible payment programs can be arranged.
Once the information is collected and examinations are performed, Dr. Allen will give you a detailed report of all findings and answer any questions including:
- How can you help me?
- How often do I need to come in?
- What will my treatment cost?
After reviewing your health history, goals, and examining your spine and X-rays, Dr. Allen will discuss* recommendations and notify you if your condition requires care with other providers. Dr. Allen will provide the best treatment and wellness program for your needs.
*Most of the time this report is presented on the second day, after Dr. Allen has a chance to study the X-rays, health analysis and examination.
